Due to the fact that we need to ensure that we are providing housing and housing support to those most in need, as well as ensuring that we meet the terms of our funders, we have set criteria in place in terms of who we are able to accept referrals from. We are currently able to work with people who meet the following criteria:

  • You must have a primary diagnosis of either a learning disability, autistic spectrum disorder or aquired brain injury (ABI)
  • You must have either a social worker or statutory mental health practitioner.
  • You must be in receipt of 5 or more hours a week of commissioned support via an external care provider (i.e. not a family member, partner or friend) and this must be in place at the point of referral.
  • You must be in receipt of Employment Support Allowance (ESA)
  • You must be able to provide proof of your National Insurance number at the point of referral

 

The Referral Form must be completed and submitted by a social worker or statutory mental health practitioner. I'm afraid that we cannot accept Referral Forms from any other individual or organisation. 

However, we are always happy to talk through our services with any person or organisation if you need any information!

In the first instance please do give us a call to discuss your referral or contact us using the online box below. After the initial conversation we will require the following paperwork to be completed:
